Important: All patients under 18 years of age must be accompanied by a parent or legal guardian at the consultation visit.

We understand that going to a new doctor’s office can be a bit confusing, but rest assured we will explain what you can expect from us every step of the way. Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Often, surgery can be performed on the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.

To allow us to serve you better on your first appointment, be sure to bring:

  • Identification
  • Insurance cards or forms provided to you by your insurance company
  • A list of any current medications you may be taking
  • Any recent x-rays if they are available

Appointments

If you have pain or an emergency situation, every attempt will be made to see you that day.

Since surgery appointments are scheduled for at least one hour a minimum, 72 hour notice is required if you wish to change your appointment. If you have any further questions, please let us know.

First Visit

Payments

Payments are due at the time of service. For your convenience, we are happy to accept Visa, MasterCard and American Express and will file all insurance on your behalf. Alternative payment methods are available upon inquiry.

Insurance

Our office participates with most major insurance carriers. In some cases, your insurance plan may require you to obtain a specialty referral form or authorization. Please contact your insurance carrier to verify our participation and the need for any special forms or authorizations. Once treatment has started, we will file your claims.

Please remember that insurance is a benefit for you or your child. If, during the course of treatment, your benefits change, the financially responsible party is accountable for all charges.
